Performance Evaluation

TO:                ASU Administrators, Department Heads, and Supervisors

FROM:          Steve A. Grant, Director
                      Human Resources Management

DATE:           April 8, 2009

SUBJECT:   Annual Performance Appraisals

The University is required to prepare a performance appraisal on each employee, annually. To satisfy this requirement, we are asking all supervisors to conduct the review session with each direct report. The current review period is from March 1, 2008 through February 28, 2009.

During the review session, performance objectives and professional development should be discussed. Each review should be signed by the employee and the supervisor. Additionally, each Vice President within the respective units should sign the reviews.

All reviews should be completed and forwarded to the Office of Human Resources Management by 12:00 noon on May 1, 2009. Mrs. Karen Clay and Mrs. Sheila Malone will be leading this project and ensuring that all appraisals are received in Human Resources.

Please retrieve the Performance Development Review forms for support and supervisory staff below.

All of these forms are the same, just different word processing formats.  Please choose the one that best fits your needs:
